Timeline: March 1 10 to August 30 10

Description:
The project is made up of three phases, together directed at training future physicians on patient education and spreading awareness on the benefits of smoking cessation.rnPhase I is composed of a 1 day training lectures on patient education and on principles of smoking cessation counseling and pharmacotherapy to the 250 house officers who will be in the selected round in their rotation & 2 day workshop where a selected group of the house officers receive training on counseling skills and approaching the smoker in the beginning of their rotation. They also receive information on how to help willing subjects quit and the referral system to the smoking cessation clinic.rnThis is followed by phase II where the trainees apply their training and counsel patients on smoking cessation throughout their 2-month rotation. They follow a standardized counseling system and keep records as they counsel. During this phase, their performance is monitored and results of their counseling sessions are recorded.rnPhase III is the final one where the training program and the performance of the trainees is evaluated. The impact on the counseled subjects is also evaluated.rnThe program is to be repeated every 2 months for a different group of house officers. Each trained group will also be followed up on a less frequent basis for the following 4 months. The best house officers to implement the program shall receive rewards at the end of their rotations.

Background:
The prevalence of tobacco use in Egypt, like other developing countries, is on the rise. The overall smoking prevalence amongst adults (15 years & older) in Egypt is 29.1%, 19.6% amongst youth (13-15 year olds) and 34.4% amongst physicians, which is way too much.rnrnThe Ain Shams School of Medicine is the third oldest and second largest medical school in Egypt. It serves more than 6000 undergraduate students over the 6 years of undergraduate teaching, and more than 1,500 post-graduate students.rnrnOur project will take place at the Ain Shams University Hospital (ASUH). ASUH is a tertiary level hospital that serves a large portion of Cairo, the Egyptian Capital. It has more than 3200 beds, with more than 10,000 staff members, and serves about 2,000,000 patients from all over Egypt annually. Even though smoking in health care facilities is prohibited by the Egyptian law, it is not enforced effectively, and even though tobacco use is a chronic health problem, it is not addressed enough by the Egyptian health-care system.rnrnThe backbone of the medical force at ASUH are the House Officers (first year post-graduation physicians). About 1,500 house officers do their internships at ASUH annually. They have a relatively low workload, a high interest in improving their clinical skills and are the ones who are in direct contact with the patients most of the time. These factors make the house officers the ones responsible for patient education and counseling, which is a central concept to the objectives of our project.
